Children's Community Residence

The Children’s Community Residence is an eight bed home located in the Baldwinsville School District. The home is certified by the Office of Mental Health. The program serves children, male and female, ages 7 to 13 that have significant mental health diagnoses, and may have experienced psychiatric hospitalizations which make remaining in their homes unsafe or challenging. The children are voluntarily placed by their parent or guardian accessing admission through the local county admission process. The Program uses the evidenced based Control to Collaboration Model which focuses on developing each child’s strengths. The Program Caseworker works closely with families so that skills can be transferred to the home setting. A Program Nurse oversees the children’s medical needs. Therapy and psychiatric services are obtained through community providers. 

Health Home Care Management

Toomey Care Management provides strength based, youth focused services to assist children that meet qualifying criteria for Health Home services to identify and access needed medical and behavioral healthcare, social services, education, financial, vocational, housing and other supports. Coordination is provided to obtain the most appropriate services to meet the needs of the child, while improving overall health and well-being and allowing children to be successful in the least restrictive environment possible. Care Managers provide oversight and coordination of a Plan of Care to ensure the delivery of high quality health and social support services.

 
 

Clinical Services

Clinical Services include Nursing case management and counseling services.

The nursing team is lead by a Nurse Practitioner who oversees Registered Nurses to manage the health care for each individual in our care. 

The counseling services are provided by Licensed Master’s level Social Workers.
